Obtaining an access card
If you are requesting an access card please deliver this form

to Genesis Falcon, ucla.xtal.facility@gmail.com in Room 106 or Duilio Cascio cascio@mbi.ucla.edu in Room 220. You must include an email address on the form that is not a UCLA address. You will be given a four number code, which is confidential. Do not share your code or card with any person or it will be revoked. The card is to be returned if you move or change positions. Report lost or stolen cards immediately to Duilio Cascio.
Obtaining a building or room key
To obtain a physical key, please download this form and submit it to Oren Saig in room 135 between the hours of 10 AM and 2 PM:
